Improving the Public Health and Quality of Life in Southern Connecticut
St. Vincent’s Community-Based Healthcare is designed to enhance the public health and quality of life in southern Connecticut and improve access to health services for members in and around the communities we serve. The program is structured in a way to deliver and sustain the desired services and outreach to those who are struggling. Our goal is to make a measurable impact on the community by extending our resources in areas that will benefit most.
As part of our dedication to the community, St. Vincent’s Medical Center provides the highest quality primary care services to children and adults of all ages whether uninsured or underinsured, low-income, disabled, homeless, frail, elderly or homebound. To perform these services, St. Vincent’s has formed a number of programs to deliver appropriate care.
